CNS: Patient is conscious and oriented x3. Head normocephalic, with normal range of motion. Deep reflexes are a +2/ normal limits. EENT: Patient’s face, eyes, and ears are symmetrical. Pupil’s size 7mm each, pupils are equal, round and reactive. Oral mucosa is dry. Skin: There is normal elasticity of the skin. Color is normal, there is tenting sign of the skin, his skin is warm and dry. MS: Upper and lower extremities have full ROM CV: Heart rhythm is irregular: no abnormal heart sounds and chest is moving equally. RCFP: Patient is in room air, there’s no coughing and pattern/effort within normal limits. GI: Patient’s abdomen is soft; bowel sounds are present x4 and able to pass flatus but patient also has diarrhea and some tenderness to the abdomen. GU: Urine is dark amber color, no odor, and no discomfort when urinating (Taylor et al., 2019).
