Cardiac Catheterization Report Patient Name: Edgar Robinson Date of Birth: 04/01/xx Patient Number: 45-67-89 Room Number: 750 October 29. 20xx Thomas Booth, M.D. 2711 Goodard Road Miami, FL 54321 Dear Thomas: Your patient Edgar Robinson underwent emergency cardiac catheterization on 10/29/xx in response to an uncomplicated non-Q, high lateral MI that clinically was moderated by medical therapy, (CPK Max 1100). Single vessel CAD with mild LV dysfunction was found. The large. bifurcated, intermediate artery was totally occluded just beyond the origin and faintly collateralized. There was only mild irregularity of the other vessels. The intermediate artery territory was moderately hypokinetic, reducing the calculated ejection fraction to 53%. Immediately following that diagnostic procedure, the intermediate was successfully stented with a 2.5 mm NIR leaving an optimal result. An echocardiogram performed just prior to the procedure confirmed only mild to moderate hypokinesis of the affected segment; i.e., he may well experience full recovery. He will require one month of Plavix therapy in addition to aspirin. He was advised to refrain from vigorous athletic activity for 3-4 weeks to allow the ventricle to recover. His LDL was mildly elevated and HDL low (26) indicating the need for aggressive dietary measures. Lipitor 10 mg 9.d. was initiated. He was referred to Dr. Dean Ornish’s book “Reversing Heart Disease” for guidance in this regard. I look forward to following along with you.
