< The team at Western States Hospital continued to work on the issue discussed in case study provided. In the months that followed, the team elected a leader, found a facilitator, and constructed its vision statement: “Patient type is correctly identified in the electronic health record at all times. It tentatively identified process customers as nursing staff, patient accounting staff, admitting staff, and patients. The team decided early in the process that it did not have sufficient data to make recommendations for process changes and decisions about system capabilities. Data collection activities were developed by the team to better understand the issues and concerns regarding patient type within the electronic health record system. This process was conducted over the past four months. Data was collected on the number of patients admitted to the emergency department with chest pain and transferred to the cath lab (a key data element for the acute myocardial infarction quality measures); examined the user procedure for entering and changing the admissions and transfers in the system; and finally, summarized the amount of admitting and transferring that each user performed. The team also conducted observational studies of the patient type transfer procedure, identifying how long the typical patient type transfer took from one patient type to another, the exact times of initiation and completion, and the time of transfer input into the information system. It identified the number and type of errors that occurred concerning patient admission and transfer, and the effects those errors had on patient care and data integrity. After the data collection and observational studies were completed by the patient type transfer team, the quality council of Western States Hospital began its annual review of the important aspects of the PI program. The leader of the patient type transfer team was asked to submit a one-page synopsis of the team’s activities that included a brief description of team activities and a complete set of the data collected by the team.
