This 72-year-old patient was admitted to this facility 2 years agodue to dementia.
Physical Examination:
HEENT: No findings
Heart: RSR without murmur
Neck: Supple with thyroid in the midline
Skin: The skin of the scalp and chest appear inflamed and red andare covered with yellow greasy appearing scales. Seborrheicdermatitis is present.
Lungs: Clear
Abdomen: Soft and nontender
Plan:
I have instructed the nursing staff to apply hydrocortisone creamto the scalp and chest areas for 5 days.
