A 45 year old physically fit patient was admitted to hospitalfollowing several visits to his General Practitioner (GP) where iswas complaining of decreased appetite, constipation, fever, chills,headaches, cramping, vertigo and respiratory problems afterexperiencing some hay fever like symptoms one week ago. He wascommenced on Roxithromycin 150mg bid and his GP ordered blood teststhat showed his had neutropenia (low white cell count) andthrombocytopenia (low platelet count). Two days later he was notfeeling any better and the GP ordered a chest x-ray (CXR) which thepatient to have bilateral pneumonia he was than admitted to thehospital.

Instructions:

1. Create a sample FDAR charting based on the case presentationabove.

2. Create 1 actual nursing care plan for your patient.

3. Create an ISBARR script for referring the said patient to hisphysician indicating:

– referring patient’s 02 saturation @ 85%
