ASSIGNMENT 3 Situation: Baby Abdulhamid, a 2 years old toddler was admitted due to difficulty of breathing. He was cuddled by her mother. There is an evident use of accessory muscle when breathing the oxygen saturation is 80%, with Respiratory rate of 50 breaths per minute, shallow. His weight is 12 kgs. The temperature is 40 degree Celsius, Pulse rate- 145 beats per min, BP of 70/30 mmHg; with sternal retractions, crackles were appreciated upon auscultation at the upper base of the lungs. The baby’s skin is pale and the lips are cyanotic. The mother claimed that 2 days prior to admission, the baby had fever which was observed as warm to touch with reddish skin, irritable, with appetite, secretions coming out from the nose. Home remedies that were done are the following- sponge bath believed to reduce body temperature; closing windows so as not to expose the baby, and steam inhalation. But still, the baby was still warm to touch, with secretions and poor suck. The mother was alarmed especially when her baby was not able to sleep the whole night and refused to eat. Hence, they consulted their pediatrician. Upon examination, the doctor advised the mother for her baby to be admitted to this institution. The following were prescribed- Acetaminophen IV 12.5 mg/kg IV every 4 hours, infuse intravenous fluid of DSIMB 500 ml to run for 8 hours, nebulization with salbutamol every 4 hours and administer oxygen at 2 liters per minute per nasal canula.
