Case study You are a RN working with a LPN and HCA. You receive the following report and 7 patients. 1) 3 day post op Knee replacement 87 yr old mild pain 2/10. Needs reassessment of pain and needs assistance with ambulation. 2) 55 yr with a fever of 39 c nausea vomiting for 3 days pain LUQ area of abdomen was admitted from ER 4 hours ago with abdominal pain NYD, he is awaiting surgery consult to assess. 3) 44 yr old admitted 3 days ago with a Ml and had 3 stents patient has physio this morning. Needs help ambulating to do his activities of daily living 4) 66 yr patient admitted 24 hours ago with Pelvic pain. Has new onset of pelvic bleeding she has gone through 2 pads every hour for the last 2 hours and BP is 80/66 pulse is 118, pt is diaphoretic. 5) 35 year admitted 2 days ago had an emergency appendectomy via Laparoscopy. Pain is controlled patient to be discharge once they have a bowel movement. No bowel movement as of yet. Pt needs encouragement to walk. 6) 20 year admitted 24 hours ago with a fracture pelvis from a ski accident. Pain is 4/10 was given Tylenol #3 3 hours ago. No other complaints. Pt has foley. 7) 33 year old patient with type 2 diabetes admitted 36 hours with DKA. On an insulin drip. On admission blood sugar was 36 mmol/L. On rounds through the night patient was vomiting all night. Given Gravol 50mg. IV. On rounds at 0600 patient still nauseated now has diarrhea and blood sugar was 6 mmol/L. Pt has had no oral intake for 12 hours due to nausea. Ringers Lactate IV running at 125ml/hour. Based on the information above, answer the following questions 1. Based on the delegation and prioritization presentation, priorize what 3 clients the RN should assess first and list in order of priority. Support with rationale from your med surg textbook. 2. Identify which clients can be delegated to the LPN 3. Identify what tasks can be delegated to the health care aide. 4. Identify 4 what questions the RN should ask themselves prior to delegating to other team members. 5. Based on the CRNM Assignment and Delegation to Unregulated Care Providers, outline the responsibilities of the RN when delegating the clients in this case study to UCP’s. Be client specific-choose 1 task you delegated to UCP’s (Health care aides) in question #3. And work through the delegation process.
