Scenario 3 – Outpatient Substance Abuse and Mental Health – DASTCarl is 45 years old with a history of heroin abuse and a bipolardiagnosis. He has had periods of stability over the years, where hehas gotten sober, has been compliant with his psychotropicmedications, reunited with his children, and obtained work as anelectrician which is a source of pride. In a recent outpatientgroup session, where Carl typically acts as a role model and mentorto younger, less stable recovering addicts, Carl becameconfrontational and moody. He interrupted fellow group members withstatements such as, “why bother then?” and “what’s the point ofeven trying?!” After this group, the social work intern asked tomeet with Carl to find out if there was anything going on that wasimpacting Carl’s typical presentation. Carl admitted that hestopped taking his mood stabilizer regularly, and had traded hismedication for some vicodin pills with a client who just had kneesurgery. Although Vicodin was not his drug of choice when he was anactive user, the intern sees this as being a slippery slope forCarl, and worries that he could have a full relapse. The internconducts the DAST based on Carl’s recent use of vicodin, and Carlonly scores a 2 (low level risk).
