Situation:
Baby Abdulhamid, a 2 years old toddler was admitted due todifficulty of breathing. He
was cuddled by her mother. There is an evident use ofaccessory muscle when breathing the oxygen saturation is 80%, withRespiratory rate of 50 breaths per minute, shallow. His weight is12 kgs. The temperature is 40 degree Celsius, Pulse rate- 145 beatsper min, BP of 70/30 mmHg; with sternal retractions, crackles wereappreciated upon auscultation at the upper base of the lungs. Thebaby’s skin is pale and the lips are cyanotic. The mother claimedthat 2 days prior to admission, the baby had fever which wasobserved as warm to touch, with reddish skin, irritable, withappetite, secretions coming out from the nose. Home remedies thatwere done are the following- sponge bath believed to reduce bodytemperature; closing windows so as not to expose the baby; andsteam inhalation. But still, the baby was still warm to touch, withsecretions and poor suck. The mother was alarmed especially whenher baby was not able to sleep the whole night and refused to eat.Hence, they consulted their pediatrician. Upon examination, thedoctor advised the mother for her baby to be admitted to thisinstitution. The following were prescribed- Acetaminophen IV 12.5mg/kg IV every 4 hours, infuse intravenous fluid of D5IMB 500 ml torun for 8 hours, nebulization with salbutamol every 4 hours andadminister oxygen at 2 liters per minute per nasal canula.
