A 72-year-old male walks in with shortness of breath &difficulty sleeping. He says that he finds that it helps when hesits up to a more neutral posture, he also mentions he hasincreased fatigue and weakness especially upon physical exertion,and he mentions that his shoes have been feeling tight over thelast little while. On physical exam, breaths sound clear but arelabored and shallow, sometimes can hear a 3rd heartsound.
