A 78-year old woman who resides in a nursing home is admittedwith a history of chronic bronchitis with an acute exacerbation.The nursing home nurse reports that the patient has had an increasein sputum production, cough, and fever. The patient has a historyof having had a stroke 2 years ago with resulting hemiparesis andaphasia. Recently, the aides have noted that the patient hasexperienced an increased difficulty in swallowing, with occasionalepisodes of choking during meals. Vital signs and assessment onadmission are as follows: 122/60-90-28; temp 38.8 C; frail, thinfemale, slightly cyanotic using accessory muscles to breathe.Diminished breath sounds in the right upper lobe with courserhonchi throughout all lung fields. Chest X-ray results; patchyinfiltrates in the right middle lobe with consolidation in theright lower lobe. CBC; mild anemia (Hgb 11; Hct 33) with anelevated WBC count (10,500).
