Case Study:
A 42-year old female with multiple sclerosis, is brought tothe Emergency Department by ambulance for fever, decreased appetitefor 5 days, with a change in mental status today. She isaccompanied by her brother, and sister in-law in which the clientlives at their residence. The client has a full code status. Thefamily reports a 5-day history of decreased appetite, and change inmental status. The family does not recall the names of the client’smedications, though the family reports that the client has beentaking medication for bladder control, eye drops for dry eye, painmedication, and a steroid on a regular basis except for the last 5days.
The nurse performs the client’s assessment and reveals thefollowing abnormal findings: Vital Signs:
• Temperature:102.5rectal
• Heartrate:115 beats per minute
• Blood pressure:90/56
• Respirations:30
• Pulse oximetry: 94% on room air
Physical Assessment:
• Unkempt with clothing soiled with both urine and foodparticles and stains
• Cachectic
• 38 kg weight per bed scale • Adventitious lung sounds
• Bounding heartrate
• Skin tenting
• Four Stage 4 wounds on sacrum and spine
o Wounds are malodorous
o Draining purulent drainage from all 4 wounds
The nurse discusses the client’s wounds with the family, andhow long the wounds have been present. The family reports that thewounds have been there for about a week. When asked who is caringfor the wounds, the family reports that they are caring for thewounds at home, without medical or nursing assistance. The nurseobserves that the wounds were open to air, without any dressings inplace. The nurse questions if the client has seen their doctorregarding the wounds, and they report that they did not call thedoctor as they were taking care of the client themselves athome.
The physician assistant (PA) discusses the client’s decliningcondition, and the need to place the client on mechanicalventilation. The PA discusses the client’s advanced directives, andthe brother informs that he is the Power of Attorney (POA) andwould like to change the client to a Do Not Resuscitate (DNR)order.
The nurse privately reports concerns to the PA regarding thenew order for a DNR order from the POA, as the nurse is reportingthe family to social services for negligence. The PA replies “WellI don’t think it was intentional neglect.” The nurse proceeds tomake the
report to social services, and contacts the hospital’s ethicscommittee regarding concerns over the family who is reported forneglect, making the decision to change the order to DNR.
