Case Scenario
Andrea, 28 y/o/F is admitted on March 20, 2021 at 3:30am to theOB ward with complaints of painful and urgency in urination. Herdiagnosis is UTI with the following orders from the physician:
Bed rest with bathroom privileges
>Obtain weight daily
>Vital signs every 4h
>Urinalysis, complete blood count
>diet as tolerated
>Start IVF of D5NaCl 1Lx8 hr; to follow the same IVF.
>Monitor intake and output
The Nurse hooked the IVF at 4am and you will be the nurse onduty at 7am -3pm.
Andrea says these are her intake:
7:00am 400ml milk
10:00am 300ml fruit juice
12:00nn 400ml blended food
12:45nn 300ml water
3:00pm 300ml porridge
5:00pm 250ml water
7:00pm 150ml water
10:00pm 100ml water
She urinated 3x at approximately 150ml every 4 hours starting7am and defecated 1x only in am.
