CASE STUDY ADPIE (Assessment, Diagnosis, Planning. Intervention, Evaluation) ASSESSMENT A 45-year-old woman comes to the clinic and asks for medication to help her sleep. She states she is worrying about too much and states. “It takes me about an hour to get to sleep, and it is very hard to fall asleep. I feel like I can’t do anything because I am so tired. My husband passed away recently. Short List of Symptoms Cluster Similar Symptoms Analyze/Interpret the Symptoms Subjective Symptoms DIAGNOSIS.NURSING Select a Nursing Diagnosis with Related Factors and Defining Characteristics P- E-related to S as evidenced by PLANNING Outcomes Nursing Diagnosis Disturbed sleep pattern related to worrying too much and death of husband as evidenced by difficulty falling asleep and statement, “I am so tired, I can’t do anything.” Client Outcome After the client outcomes are selected and discussed with a client, the nurse plans nursing care and establishes a means that will help the client achieve the selected outcomes. The usual means are nursing interventions NURSING INTERVENTIONS Nursing interventions are like road maps directing the best ways to provide nursing care.
