Emily, age 40, after being treated for razor cuts to her arms and legs, has been admitted to the psychiatric inpatient unit. During the first week of hospitalization, Emily emotionally attaches herself to several staff members. By the end of the week, Emily states that she hates several of the same staff that she once adored and attempts to pit one against the other. When Scott, a psychiatric nurse confronts Emily about trying to sneak alcohol onto the unit, Emily seductively says, “Why don’t you come to my room tonight and I will help you forget about this incident”. History reveals a poor relationship with her mother, and no relationship with her father, who deserted the family when she was a toddler. Angrily, Emily states “I think she drove him away. My mother only cares about herself.” Emily, although well-educated, demonstrates poor work ethics. Over the past 10 years, she has been fired five times due to being late or absent, and using drugs and alcohol during work. Emily can not maintain any friendships on the unit because she is easily bored and gets angry when she is not the center of attention. Emily admits to feelings of depression and anxiety and tearfully states that cutting and alcohol somehow make her feel less hollow. She has an angry affect and eye contact that can be absent or glaring, depending on her mood.
