(why does the patient need to get this testing done? What is being measured etc.) Each lab value must be accompanied by the following: 1. Normal range 2. Indications (meaning what are we trying to evaluate) 3. Description of the lab value 4. What would cause an increase in levels? 5. What would cause a decrease in levels? Please keep in mind 1. Sources must be cited for each lab value 2.
