Sarah James, a 8 year old female was admitted to the unit forleukemia. She was stabilized and getting ready for discharge.
Her nurse felt ill during the shift and was taken to the ER.Nurse Isabel was floated to the unit to replace the nurse, uponassessment Sarah James, appeared to have a unit of bloodtransfusing and complains of itching and a rash on her back.
The nurse had no report on the transfusion and she went to thechart to identify the order and there was no consent or order forthe transfusion.
