“Thump” a 15 week old 16 lb puppy runs into a wall head first and is knocked unconscious. His owners bring him in because he is not acting just right.” On physical examination, he is depressed, lying on his side and stares at the wall and seems uninterested in anything you do. He does not menace or track when you test him, and has a normal but slow PLR. He barks at the floor when you clap your hands. He can stand if placed in a standing position but immediately lies back down on his side. His spinal reflexes are normal. All vital signs are within normal limits and no external injuries are found on the puppy. Modified Glasgow Coma Scale June 7, 2010 (published) Modified Glasgow Scale-Platt et al, JVIM 15:581-584, 2001.) 3-8: Grave • 9-14: Guarded 15-18: Good In this study, if the animal developed a score of 8 or less, they were not likely to survive the first 48 hours. 6 5 4 3 2 1 6 5 Motor activity Normal gait, normal spinal reflexes Hemiparesis, tetraparesis, or decerebrate activity Recumbent, intermittent extensor rigidity Recumbent, constant extensor rigidity Recumbent, constant extensor rigidity with opisthotomus Recumbent, hypotonia of muscles, depressed or absent spinal reflexes Brain stem reflexes Normal pupillary light reflexes and oculocephalic reflexes Slow pupillary light reflexes and normal to reduced oculocephalic reflexes Bilateral unresponsive miosis with normal to reduced oculocephalic reflexes Pinpoint pupils with reduced to absent oculocephalic reflexes Unilateral, unresponsive mydriasis & reduced to absent oculocephalic reflexes Bilateral, unresponsive mydriasis & reduced to absent oculocephalic reflexes Level of consciousness Occasional periods alertness and responsive to environment Depression or delirium, responsive, but response may be inappropriate Semicomatose, responsive to visual stimuli Semicomatose, responsive to auditory stimuli Semicomatose, responsive only to repeated noxious stimuli Comatose, unresponsive to repeated noxious stimli 4 3 2 2 1 ula 4 3 2 1 Answer to 13th question: The breed of the Thump dog is Irish setter breed which is a family dog. Answer to 14th question: Based on Modified Glasgow coma scale. the normal spinal reflexes has score 6, Thump shows slow pupillary light reflexes PLR has score 5 and thump is depressed. responsive but the response is inappropriate has score 5. Then the total score = 16 which based on Modified Glasgow coma scale shows Good condition. Answer to 15th question: Furosemide is the diuretic the veterian most likely order for Thump dog. Answer to 16th question: Given dosage = 0.5g/Kg Thump body weight = 16 lb weight change into Kgs: 16 lb = 7.257Kg then 0.5 gram of drug equals to 1 Kg then 7.257 Kg = ? then (7.257 x 1) / 0.5 = 7.257 /0.5 = 14.5 gram Drug diuretic dosage = 14.5 grams Conversion of grams into Milligrams 1 gram = 1000mg 17. When you go to get the bottle of this diuretic, you find it has crystallized in the bottle. You check and it has not outdated. What should you do? 18. What laboratory values should be monitored in Thumper while he is receiving this diuretic and why? 19.* Buprenorphine(0.3 mg/mL) is also ordered for Thumper at a dosage of 0.015mL/kg. Calculate the dose and write it as though you were writing it on the record. (1.5 pt) = 20. Why was buprenorphine ordered? 21. Do you think there is any specific way you should place Thumper in his cage?
